在唐朝懿宗皇帝时，有一位知玄悟达国师，在他年少还未被封为国师时，曾参访丛林，挂单在一间不知名的寺里，正巧另一位僧人也挂单在该寺，但那位僧人得了很重的病，通身长满了疮，发出很难闻的臭气，所以都没有人想要和他来往，国师住在他的隔壁，很同情他的病苦，常常照应他，一点都没有讨厌躲避的感觉。
不久那位病僧的病也好了，为了道业各奔前程，在临别的时候，那位僧人为了感激知玄和尚的德风道义，就对他说：「你以后如有难临身，请你不妨到西蜀彭州九陇山来找我，我会设法解救你的灾难。记住山上左边两棵大松树连在一起，那就是我居住地方的标志。」说完便离去了。
后来知玄和尚因为德行高深，唐懿宗十分崇敬，就封他为悟达国师，还赐他沉香庄饰的宝座，悟达国师坐上宝座之后却生起一念傲慢心，心想现在是一人之下万人之上，于是从这时候开始，膝盖上便生出一个人面疮来，长得和人面一模一样，每次还要用饮食喂他，也能像人一样开口吃东西，悟达国师当时痛苦难忍，遍请各地的名医，但是每位名医都无法医治。
国师这时突然记起过去，同住的那位病僧临别所说的话，于是便前往西蜀彭州九陇山去寻找。一日，天色已晚，忽然看见了两棵并立的松树，再往前一看是一座金碧辉煌的殿堂与那僧人，两人相见甚欢，国师便把所患的怪疾告诉他，僧人便加以安慰，告诉国师不要担心，只要用清泉水洗一洗即可。次日清早僧人就令一个孩童引领国师到岩下清泉的溪旁清洗，国师刚要捧水洗人面疮时，人面疮竟然大声呼喊：「不可以洗啊！您知识广博、见解深远，但不知是否曾读过西汉书上，袁盎与晁错传呢？」国师回答说：「曾经读过。」人面疮就说：「往昔的袁盎就是您，而晁错就是我，当时晁错被腰斩时，心怀怨恨，因此我累世都在寻求报复的机会，可是您十世以来，都是身为持戒严谨的高僧，冥冥中戒神在旁守护，使我没有机会报复，而今您受到恩宠，动了一念名利心，无形中德行已经亏损，因为这个缘故，我才能接近您的身边来报仇。现在蒙圣人迦诺迦尊者出面来调解，赐我三昧法水（此三昧水是迦诺迦尊者三昧力的加持而成三昧法水），让我得解脱，今后我不再与您为难作对了。」
悟达国师蒙受圣人的救护，洗除多生的宿冤，因此写成忏本，早晚恭敬礼诵，来报答大圣的恩德，并命名为慈悲三昧水忏，就是取尊者以三昧水洗人面疮，解开两家累世冤业的缘故。
今日大众在礼拜忏悔时，藉由祖师大德身行、言教的提示，应当发起真诚的清净心，以三昧法水洗去我们累劫的尘垢。
