嘉義市已連續4 年人口減少，老年人口比例逐年增加，依2015年8 月統計資料顯示，嘉義市總人口數為270,656 人，其中65 歲以上老年人口已達33,860人，占全市人口12.51%，較全國12%來得高，因此，對於老化之相關問題，市府在政策將朝向更具前瞻性的作為前進，最具體的項目即為推動『家庭責任醫師制』（即「家家都有醫師兒計畫」政策），積極建設嘉義市成為健康防老城，從觀念面、軟體面下手，建立正確健康促進、健康防老觀念，扭轉現行醫療體制，依法（健保法第44 條）推動家庭責任醫師制，希冀運用智慧行動裝置、結合雲端資訊科技管理為時代潮流，未來居民的預防保健與健康促進服務，將朝智慧型雲端資訊管理系統方向發展。事實上，當高齡化，少子化已經是全世界的趨勢，未來的社會將隨著戰後嬰兒潮退出職場而使得照護人力越來越不足。要解決高齡化少子化社會之問題，就必須運用資訊科技的力量，強化照護體系的能力與能量。並且，這樣的能力與能量必須從醫院之病人延伸到健康人，才能夠降低醫療資源佔國家支出之比例，同時也提高照護品質。另一方面，面對全球的健康醫療事業的高度挑戰，醫療健康產業勢必快速的轉型，民眾也必須以更積極的對自己身體的健康管理負責。然而，醫護資源卻因為整體健保制度，以及少子化影響，而呈現醫護人員出走或供應萎縮的情況；當需求與供應無法適當對應時，唯有同時在需求端與供應端上進行處置，才有可能紓緩類此捉襟見肘的冏境。
就需求端而言，應設法延長民眾健康的時間，即透過各種健康促進與日常健康管理方案，減少生病機率，即使不幸染病，也能及早發現，並在體力狀況較佳的情況下抑止病情惡化，以確保醫護需求量得以下修；在供應端方面，則應設法降低醫護負擔，讓有限的資源發揮最有效益的地方，例如：減少不必要的檢查、避免重覆用藥，或者導入其他醫護專業應用服務機制，如：專屬家庭責任醫師、遠距健康照護服務中心，讓民眾發生健康疑慮的第一時間先行諮詢可靠的專家意見，而不是不分輕重緩急直奔大醫院，造成各大醫院診間人滿為患的情況，亦讓健保醫療支出連年攀升。
隨著科技的帶動發展，健康照護服務的模式也隨之改變，線上諮詢、健康運動、飲食、衛教等互動應用亦隨著各種不同軟硬體、基礎建設的成熟蓬勃發展，也因此讓許多的病患可以從實體的醫院治療延伸至家中的照護，同時也驅動更多的一般使用者透過不同的穿戴式或是行動裝置結合相關應用服務，建立個人化的健康管理模式與健康生活型態，也提升了智慧化的生活品質。
