據《藥師琉璃光如來本願功德經》記載，藥師佛在因地修行菩薩道時，曾發十二大願：

二、願我來世得菩提時，身如琉璃，內外明徹，淨無瑕穢；光明廣大，功德巍巍，身善安住，焰網莊嚴過於日月；幽冥眾生，悉蒙開曉，隨意所趣，作諸事業。
三、令諸有情悉皆圓滿；
四、令一切眾生行者安住大乘法宗；
五、令諸有情修行梵行如法清淨；
六、諸根不具者皆得端嚴；
七、令諸有情身心安樂乃至菩提；
八、由女身轉男身；
九、令諸有情消除外道邪見修習諸菩薩行；
十、令諸有情解脫一切憂苦；
十一、令諸有情隨意飽滿令住安樂；
十二、令諸有情得種種上妙衣服無諸苦惱。
