We have a history of caring for people. Dr. Robert C. Banasik has been in the nursing assisted living and independent living profession for 36 years. We have been entrusted with the care of someone like you and or your loved ones. We know the heart break of having a loved one in need. One of the first concerns of residents is the lack of control over their environment. Leaving their home of many years is among the first losses. Throughout our years of service when the doors of life are closing we pride ourselves on keeping the windows open wide. This has been accomplished by maintaining our stewardship in small but important ways. One of the most important things people can still do when they have problems is to make decisions. Food by choice rather than by the dietary department’s selection is but one small example. As most of you are aware, it is the little things such as sand in your shoes that wear you out.To that end, we have developed our patent-pending system to foster choice in a unique home environment. Throughout our years of service we have seen the dread of needing help in any way. However when a little help is all you need we have found a special way of providing it. Historically, many of our visitors have said that they wish they could do more. Now they can. Our program helps people on a one to one level. It is a way of combining people who want to care for others with folks who are looking for a little help.
This matching is provided by the wonders of the internet with combined with committed talented people. Essentially, you tell us what you need and we will match you to people in the area you select. They will then provide you with a little help that you need. Caring for folks on a one to one basis means there is a natural consistency of care by the nature of the relationship that develops between the host and guest.
Our goal is to provide you the host or the guest with a convenient way to find each other. The first step in that process is to complete a functional care survey to assess your recommended level of care. This app is the questionnaire. You can answer each question. You can email the results of your questionnaire. You receive in the email your recommended level of care, such as independent, assisted living, or skilled nursing. The email contains your completed survey and next steps.
You can download the app. You can open the app. The questionnaire shows. You can tap your answers. You can tap Email Results. You can fill in Name and Email. You can tap Send. Care&Host determines your recommended level of care based on your answers. Care&Host emails the results.
Care&Host uses decades of experience to determine your recommended level of care. The topics on this questionnaire are often broached to an individual for the first time at a skilled nursing facility in the pressure and newness of a dedicated care facility. This app enables the questionnaire to be completed wherever and whenever you like. Completing the questionnaire is the first step in the patent-pending Care&Host process. The emailed results include the recommended level of care based on your answers, your completed questionnaire with each answer, and next steps.
You can complete the questionnaire as many times as you like. You can email as many questionnaire results to as many people as you like.

If you are interested in helping, a graphic shows you how much you can receive in rent. You can complete the assessment questionnaire and email your results.
