La Real Fundación Victoria Eugenia, promueve la publicación de la guía "Recomendaciones para el tratamiento de la Hemofilia con Inhibidor".En los últimos años estamos asistiendo a un avance sin precedentes en el tratamiento de los pacientes con hemofilia. En los pacientes con hemofilia e inhibidor estos avances son especialmente relevantes y accesibles en el futuro inmediato para la rutina diaria. Se ha establecido una nueva clase de agentes terapéuticos que actúan mejorando la coagulación (emicizumab) e inhibiendo las vías anticoagulantes (fitusiran y concizumab), que funcionan como productos útiles en la profilaxis de los sangrados en este grupo de pacientes. Sin embargo, aún se precisa información sobre su seguridad a largo plazo y manejo de las posibles complicaciones de su empleo. Además, necesitamos implementar técnicas de laboratorio que permitan evaluar la función hemostática de los pacientes tratados con estos productos y su combinación con agentes baipás y factores VIII y IX en caso de cirugía o sangrado agudo. Finalmente, se abren otras áreas de evaluación como son el papel de la inmunotolerancia en la actualidad.
En la siguiente guía se pretende revisar las nuevas perspectivas del tratamiento de los pacientes de inhibidor en lo referente tanto a la prevención y tratamiento de los sangrados como a los episodios quirúrgicos. El objetivo es que sirva de ayuda en la indicación, monitorización y evaluación de estas nuevas terapias.
