web and mobile products enable the assigned caregiver to securely track and report
daily care plan tasks that are completed back to the to the health plan and the responsible care plan managers
Our GPS monitoring capabilities, validates that care had been delivered to the approved
health plan member. The enables the health plan to follow the covered member
throughout the continuum of care from acute settings and back into the home.
Provider Case Manager Dashboard: Health plan case managers can access CareHeroes caregiver reporting platform in real time and review caregiving tasks completed by paid caregivers delivering care to their members. A secure messaging/chat platform also enables a care managers to easily communicate with identified caregivers and provided much needed support and advice.
