- Manage your important contact information and set preferences through My Profile.
- View personal medical history, get your primary care doctor’s information and search the entire network by city or specialty in My Health Record.
- Access important phone numbers and locations for all of the services you have access to with Health Plan of San Joaquin like offices near Stockton and Modesto, a 24/7 Nurse Advice line, vision care through VSP and mental health services.
