We would like to encourage patients and carers to use this application to access to contact information of patient self-help groups anywhere and anytime for sharing experiences and care tips with peer.The Smart Patient Website (SPW) (http://www.smartpatient.ha.org.hk) was launched in 2009. It is a one stop information website especially designed for patients and carers to access accurate information which enhances their knowledge of disease and community resources, so as to improve their self-care skills and strengthen their ability on disease management.
