Description
En esta aplicación se calculan los requerimientos insulìnicos que precisan pacientes hiperglucemicos hospitalizados. Se proponen dos règimenes terapèuticos y vìas de administración diferentes. Insulinización intravenosa, recomendada en pacientes criticos inestables (UCI), y subcutanea -pauta intensiva-, en aquellos pacientes estables que toleran la ingesta oral o nutrición enteral. Para la elaboraciàn de estas calculadores me he basado en el algoritmo del ensayo clinico NICE-SUGAR , brazo del estudio que demostró una disminución en la mortalidad de pacientes ingresados en UCI si se mantenian niveles de glucemia comprendiodos entre 140-180 mg/dl. En pacientes estables, he seguido las recomendaciones clásicas que demuestran que los regimenes intensivos de insulina subcutanea, que tienen el objetivo de alcanzar cifras prepandriales comprendidas entres 90-130 mg/dl, mejoran el pronòstico tanto de los pacientes diabéticos como en los no diabéticos. El ajuste de dosis de insulina intravenosa tiene encuenta la tendencia (ascendente o descendente) producida en la última hora de tratamiento y la cantidad de insulina responsable de esta variación. Se adjunta bibliografia y fórmulas utilizadas en el càlculo y ajuste de dosis.Latest updates
