Description
De Noord-Holland Zuid (NHZ) Connected app is ontstaan uit een initiatief van het Medisch Coördinerend Centrum (MCC) Haarlem en Meer. Het idee van de app is dat zorgverleners in de regio (1e en 2e lijn) elkaar makkelijker kunnen vinden en bereiken om de onderlinge samenwerking te bevorderen. De app en bijbehorende website (www.nhz-connected.nl) zijn hiervoor het regionale informatieplatform.Elke deelnemer heeft een eigen persoonlijke account in de NHZ Connected app en kan deze aanvullen met specifieke informatie zoals een direct telefoonnummer, werktijden, vakantie en/of een specialisatie.
In deze app vind je naast onderlinge contactinformatie ook informatie die bijdraagt aan het versterken van de samenwerking in de keten, zoals bijvoorbeeld regionale transmurale afspraken (RTA) of een sociale kaart.
In de komende periode wordt de app steeds verder voorzien van nieuwe functionaliteiten.
