With the Heart Center Home Monitoring Program, Boston Children’s Hospital Heart Center patients can send clinicians key clinical information, including weight, oxygen saturation, formula/fluid amounts and other important information that can help improve your care. Simply send information from home or on the go using any smart device. Your team at Boston Children's will review this information to monitor and improve health, decrease trips to the hospital, and give you more control over your care or your child’s care.The app also includes an intuitive graphical interface that lets you chart progress over time. You can view the same data and graphs as your care team and discuss results with them over the phone or in person.
You can also review key information about the Home Monitoring Program, including when to call your care team. The app contains key contacts and phone numbers.
Heart transplant patients can also see information about anti-rejection medication, including drug levels and doses. Graphed on an intuitive, user-friendly display, the program shows when drug levels are falling within the goal range and ensures that you and the care team share the same understanding of current drug dosages. This allows you to become more actively involved in care, prevent rejection, and minimize drug side effects.
