Effective January 1, 2014 – United co-workers enrolled in an Aetna medical plan such as HDHP, PPO, EPO have access to a special network of health care providers. Called SmartCare, the network includes primary care physicians, specialists, hospitals and urgent care facilities in Chicagoland and the Greater Houston Area from your Andriod device. When you use SmartCare doctors and hospitals you will have lower out-of-pocket costs and access to some of the best doctors and hospitals. Bottom line, through SmartCare, United has arranged for access to some of the highest quality health care at a reduced cost and at the same time provides co-workers with better benefits.Once the FREE smartphone app is downloaded, just enter your ZIP Code and you can begin searching for providers or facilities. You will be able to locate addresses, maps, and phone numbers for all provider types.
To use this app you must be enrolled in an Aetna PPO, EPO, or HDHP plan offered by United.
Co-Workers who do not live in Chicagoland or the Greater Houston Area may also have access to SmartCare in-network providers in those areas. To ensure you receive the enhanced benefit, print a copy of the SmartCare Guest Card and present it during your visit with your Aetna ID card. If you do not have a SmartCare Guest Card, you can find and print one using this link: www.aetna.com/united.
