Description
CONNECT is een Medisch Coördinerend Centrum in de regio Rivierenland, opgericht als stichting door de Coöperatieve Huisartsen Vereniging Gelders Rivierenland, Ziekenhuis Rivierenland, en de medische staf van Ziekenhuis Rivierenland. CONNECT heeft als doel de samenwerking in de zorg te bevorderen en daarmee een bijdrage te leveren aan betere afstemming tussen de zorgverleners in de eerste en tweede lijn. Deze CONNECT app is als middel ontworpen om hier een bijdrage aan te leveren, en is bedoeld voor alle huisartsen, medisch specialisten, specialisten ouderengeneeskunde en apothekers in de regio. De app is als presentje gekoppeld aan de Sandwichcursus 2014.De CONNECT app bevat een smoelenboek (wie is wie) met daarin de contactgegevens van de huisartsen, medisch specialisten, apothekers en specialisten ouderengeneeskunde in de regio Rivierenland. Tevens zijn alle werkafspraken (Regionale Transmurale Afspraken) van CONNECT in de app te vinden, welke zijn opgesteld door huisartsen en medisch specialisten tezamen. Er kunnen werkafspraken gezocht worden op categorie, op alfabet, en door middel van een algemene zoekfunctie. Ook bevat de app de laatste nieuwsbrieven van CONNECT. Voor het smoelenboek hebben gebruikers een inlogcode en verbinding met internet nodig. De werkafspraken en nieuwsbrieven zijn voor iedereen zichtbaar en offline beschikbaar. De inhoud van de app wordt regelmatig geactualiseerd.
Er is nu ook een onderdeel voor Nascholing toegevoegd.
