เนื่องจากในปัจจุบัน สถานปฏิบัติการเภสัชกรรมชุมชน ( โอสถศาลา ) ได้จัดให้มีบริการดูแลการใช้ยาของผู้ป่วยโรคเรื้อรัง ( medication Therapy Management ) เพื่อค้นปัญหาและแก้ปัญหาในการใช้ยาของผู้ป่วยโรคเรื้อรัง ผู้ป่วยที่ต้องใช้ยาหลายรายการ หรือกลุ่มยาที่ต้องเฝ้าระวัง ทั้งนี้เพื่อให้ได้ผลตามเป้าหมายการรักษา ลดการเกิดอาการไม่พึงประสงค์และลดปัญหาจากการใช้ยาและผลิตภัณฑ์เสริมอาหารอื่นๆ ซึ่งในการดำเนินการปัจจุบัน การเก็บประวัติเหล่านี้ได้จัดทำเป็นเอกสารแฟ้มประวัติผู้ป่วย ( Patient Profile ) และมีบันทึกหรือสมุดประจำตัวผู้ป่วยให้ไว้กับผู้ป่วย เพื่อใช้บันทึกผลการติดตามดูแล แก้ไขปัญหาโดยเภสัชกร ตลอดจนการบันทึกค่าผลตรวจต่างๆเมื่อผู้ป่วยได้รับการตรวจจากโรงพยาบาล และสมุดบันทึกประจำตัวเล่มนี้ผู้ป่วยจะต้องนำมาทุกครั้งที่มาปรึกษาที่สถานปฏิบัติการ ฯ ( โอวถศาลา ) และนำไปแสดงให้แพทย์ผู้รักษาได้พิจารณาผลของการใช้ยาและปัญหาต่างๆในช่วงระยะเวลาของการรอพบแพทย์แต่ละเดือน ในการนี้..นิสิตปี 6 ที่ฝึกงานที่สถานปฏิบัติการเภสัชกรรมชุมชนได้ร่วมกันคิดนวัตกรรมในการจัดทำ แอพพลิเคชั่น “PharmCare Note” ขึ้นแทนสมุดบันทึกประจำตัวผู้ป่วย (ที่เป็นเล่ม) เพื่อให้เกิดความสะดวกในการบันทึก ค่าความดันโลหิต หรือค่าน้ำตาลในเลือด โดยที่เภสัชกรที่ดูแล สามารถติดตามผลค่าต่างๆโดยผู้ป่วยไม่ต้องมาที่ร้านยา ตลอดจนการสรุปรายงานปัญหาและการแก้ไขโดยเภสัชกร ทั้งนี้ผู้ป่วยสามารถเข้าถึงข้อมูลด้านสุขภาพของตนเองได้ตลอดเวลา สะดวกต่อการพกพา และใช้เป็นสื่อกลางในการสื่อสารระหว่างบุคลากรทางการแพทย์และผู้ป่วยส่งผลให้ผู้ป่วยได้รับการดูแลเรื่องยาและใช้ยาได้อย่างมีประสิทธิภาพตรงตามเป้าหมายของการรักษาHow to install PharmCare Note APK on Android phone or tablet?
