Se trata de una aplicación sencilla que calcula la puntuación MADRID-DIRECT para ayudar en la toma de decisiones de la atención prehospitalaria del ictus agudo desarrollada para la Red de Ictus de la Comunidad de Madrid.La escala MADRID-DIRECT se basa en los items de edad, tensión arterial sistólica, y cinco items clínicos sencillos:
- Desviación de la mirada conjugada
- Alteración del lenguaje: si el paciente no obedece órdenes sencillas
- Heminegligencia (en pacientes con lenguaje conservado): si el paciente no reconoce su propia extremidad cuando se le pregunta "¿de quién es este brazo?" o no reconoce su déficit cuando se le pregunta "¿puede mover bien los brazos?"
- Paresia de la extremidad superior: si el paciente no vence gravedad con dicha extremidad (cae contra plano a la movilización pasiva)
- Paresia de la extremidad inferior: si el paciente no vence gravedad con dicha extremidad (cae contra plano a la movilización pasiva)
El resultado del cálculo orienta a la necesidad de traslado del paciente a la Unidad de Ictus más cercana o bien de traslado directo al Centro de Intervencionismo correspondiente para valoración de tratamiento endovascular del ictus.
El uso de esta aplicación se reserva a personal sanitario cualificado en el marco del Plan de Atención al Ictus de la Comunidad de Madrid.
