Early Onset of Sepsis atau Sepsis onset awitan dini dapat menyebabkan morbiditas yang fatal pada neonatus dengan insiden 0.77 kasus /1000 kelahiran hidup pada tahun 2005-2008. Tata laksana sepsis awitan dini yang direkomendasikan oleh American Academy of Pediatrics (AAP) adalah memberikan antibiotik pada bayi baru lahir dengan dugaan sepsis awitan dini tanpa melihat parameter klinis. Terapi antibiotik memiliki efek samping yang signifikan jika diberikan di minggu awal kehidupan antara lain perubahan mikroba intestinal, sepsis awitan lambat, enterocolitic necrotican serta meningkatnya resistensi obat pada kasus sepsis dan meningkatnya infeksi jamur (Brecht et al., 2009).Penelitian di RS Dr. Soetomo menunjukkan proporsi penggunaan antibiotik yang tinggi dari pasien di rumah sakit yaitu sebanyak 84% pada tahun 2003. Penggunaan antibiotik yang tidak tepat sangat tinggi, dimana 42% tidak ada indikasi pemberian antibiotik. Hasil surveilance pada tahun 2012 di RS Dr. Soetomo menunjukkan penurunan penggunaan dari antibiotik yang tidak tepat, namun terdapat peningkatan prevalensi extended-spectrum b-laktamase (ESBL) klebsiela pneumoniae, Escherichia coli serta methicillin-resistant S.aures (MRSA) (Usman Hadi, 2013). Di unit Neonatologi RSUD Dr. Soetomo berdasarkan laporan peta kuman tahun 2016 terdapat 41% klebsiella pneumonia diikuti dengan Acinetobacter baumanii sebanyak 26%. Berdasarkan pola sensitivitas antimikroba Coagulae-negative staphylococcus kurang dari 10% bayi yang masih sensitive terhadap Ampicillin sebagai salah satu anyibiotik empiris spektrum luas.
