Description
With the donations from The Hong Kong Jockey Club Charities Trust, the Jockey Club School of Public Health and Primary Care, the Chinese University of Hong Kong, St. James' Settlement, Christian Family Service Centre, United Christian Nethersole Community Health Service, and Hong Kong Young Women’s Christian Association co-organise a project titled "Jockey Club We WATCH Healthy Lifestyle Project" (“The Project”).The Project aims to facilitate middle-aged people, especially working class, to improve their lifestyle to become more healthy one, and then to prevent any possible chronic diseases risks Through developing and enhancing the awareness of the benefit of lifestyle medicine and health coaching.
The Project is planned to be carried out from 2023 to mid-2025. The target audience are local middle-aged participants aged 35 to 59 who are not currently members of the District Health Centre and are with potential health risks. Interested participants are invited to do a screening survey about health risk. After conducting the screening survey, preliminary eligible participants will undergo a more detailed health questionnaires on exercise science, nutritional science, emotional and mental health, etc., and have a blood test. After conducting the health questionnaires and blood tests, eligible participants will be invited to participate in the six-month health coaching provided by the above-mentioned NGOs.

「賽馬會 We WATCH 優活健康計劃」希望協助中年人士，改善生活習慣，預防或延緩患上慢性疾病。計劃是全港首個使用「生活方式醫學」模式幫助公眾預防慢性疾病的公共衛生項目，於2023年至2025年期間進行，目標招募超過20,000人參與篩查，並為合資格人士提供健康管理指導。
合資格參加者將可接受為期6個月、採用「生活方式醫學」作介入模式的個人化健康管理指導，在健康管理教練的指導下培養健康生活的意識，學習維持健康生活習慣的方法，並持之以恆，長遠實踐「預防勝於治療」。
本計劃承蒙香港賽馬會慈善信託基金慷慨捐助，由香港中文大學賽馬會公共衞生及基層醫療學院聯同四間本地非政府社會服務機構─聖雅各福群會、基督教家庭服務中心、基督教聯合那打素社康服務及香港基督教女青年會攜手舉辦。
計劃內容：
1）免費血液檢測（血糖及血脂水平）
2）私人健康教練提供個人化健康管理指導
3）提供運動健康手錶，量度運動水平，了解自身健康狀況
4）社會服務機構提供健康講座
計劃為期6個月，費用全免。完成計劃後，更有機會獲得運動手錶及健康報告。
