2 ★TONS of potential here but buggy & frustrating to set up. Love organization of interface, but data input & saving needs to be more user friendly.

1 ★Doesn't work on Droid X. Seems like it would be a good app to have if it would work. Also the icon is pretty ugly. Sorry. Pls fix & ill use.

1 ★When trying to enter information, presented with blank screen with no input fields. Trying to save crashes the app. Doesn't work at all. N1, Froyo.

4 ★Won't let me put anything in the organ donor field (should just have yes or no radio buttons). Keeps wanting to put my address there instead.

2 ★Incomplete and buggy. Forms are not spaced correctly on my phone on the contact pages.
