唐朝时称西方、北方各民族为胡人，胡琴为西方、北方民族传入乐器的通称。至明清时期，胡琴成为弓弦乐器的通称。
弓弦乐器至最早在唐朝出现，可能是从弹拨乐器筝发展出以竹片拉奏的轧筝。
宋朝陈暘《乐书》为胡琴最早之文献记录。
五四运动时期，刘天华融合欧洲的音乐理论、演奏技巧，根据中国民间音乐特性及借镜小提琴，将二胡定弦为d、a，并对二胡的演奏技巧进行革新，扩展二胡的表现力，并且为二胡创作多首独奏曲。
1920年代，周少梅、周荣根、陶洪茂合作改良二胡：加长琴杆至90公分、加大琴筒、改用莾蛇皮、改用软弓、改用更粗的琴弦，使音量、音色有所改善。
1930年代，蒋风之设计出「六角哑铃式」二胡，又被称为「蒋氏南胡」。周荣庭与蒋风之、陆修堂等共同设计出「洋山柄头」二胡和「迴纹头」二胡。
1950年代至1960年代间，上海民族乐团、中国广播民族乐团、中央民族乐团等模彷交响乐团的职业「民族乐团」出现，增加对二胡专业的需求。
1963年，首届全国二胡比赛于第4届上海之春音乐节举办。
1970年代，汤良德研製出前八角后圆（方圆）二胡和可调节的半月形音窗。1975年，王国潼与满瑞兴製订出「扁八」二胡的尺寸为：琴筒长133毫米，前口纵向直径86毫米，横向直径92毫米，后口内径84毫米；琴筒中加装竹筒以改良高音音色。
1970年代，王国潼与满瑞兴开始研发二泉琴，1980年代闵惠芬与王瑞泉、沉德庆研製成功。
现代二胡作品出现，有《豫北叙事曲》(1959年)、《三门峡畅想曲》(1960年)、二胡协奏曲《长城随想》(1982年)等。
1980年代中后期，已出版的二胡独奏曲达数百首，并开始发展叙事曲、协奏曲等大型二胡曲。
台北市民族器乐大赛、中央电视台民乐大赛等专业比赛相继举行。
